Lost Licence & Change of Name/Address Form
Have you recently moved? Have you recently had a name change? Do you require a new licence with the correct information on it?
- Click on the link above to print the Lost Licence & Change of Name/Address form.
- Complete the form and include your original licence, and send to the OFSC Driver Training office. (Your Change of Name/Address Replacement Licence will be free of charge only if your original Motorizes Snow Vehicle licence is included with the proper form.)
Have you lost your Motorized Snow Vehicle Operators Licence? Did it go through the wash? Has it been stolen?
